Key Points:

1. Referral Only for Agencies: The form is for agencies or organizations referring on behalf of a client. If you’re the individual who needs assistance, you’ll have to reach out directly via email at [email protected] or by calling 0121 661 5050

2. Completeness of Form: As with the previous form, all sections must be filled out. If a section cannot be completed, provide an explanation. Blank sections won’t be accepted, so it’s important to give enough information where required.

3. Data Sharing: The form outlines that your data may be shared with:

   • Local authority Housing Benefit departments

   • The referral agency that directed you to Sahara Housing Association (e.g., Social Services, Health)

   • Other organizations under the Sahara Housing Association umbrella, and potentially Police, Courts, or Probation, if necessary and legally required.

4. Legal Obligations: Your data might be shared in limited circumstances, such as when it’s required for legal proceedings or when complying with a court order or government authority. Sahara Housing will try to inform the individual when this happens, unless restricted by law.
